Skilled Professional Services

Skilled professional services include skilled nursing services, physical therapy, speech-language pathology services, occupational therapy, physician services, medical social work services.

Skilled professional services are authorized, delivered and supervised only by health care professionals who meet the appropriate qualifications specified under CMS 484.115 (personnel qualifications) and who practice according to the Agency’s policies & procedures.

Skilled professionals must assume responsibility for:

  • Ongoing interdisciplinary assessment of the patient
  • Development and evaluation of the plan of care in partnership with the patient,
    representative (if any), and caregiver.
  • Providing services that are ordered by the physician as indicated on the plan of care.
  • Patient, caregiver & family counseling.
  • Patient & caregiver education.
  • Preparing clinical notes.
  • Communication with all physicians involved in the plan of care and other health care
    practitioners (as appropriate) related to the current plan of care.
    Participation in the QA Program.
  • Participation in Agency sponsored in-service training.

Skilled Nursing Services

Professional nursing services are provided in accordance with the plan of care and are case managed by licensed Registered Nurses who may also be the visiting nurse assigned. Additionally, case managers may assign LVN’s who service cases under the supervision of the RN and assist in the implementation of the plan of care.
